The debate about the benefits (harms) of canceling the internship has been going on for years. Supporters of the reform of medical education believe that the abolition allows you to start working earlier, help get rid of the shortage of personnel, and increase the level of knowledge of the young doctor. Opponents have their own arguments: the reduction of the theoretical part will not be beneficial, there is no practice of communicating with the patient.

Expectations of medical students to return internships in 2022-2023 are likely to be disappointed as there were no changes in the previous 2022-2023 academic year. After the institute, in-depth knowledge of the speci alty is obtained in residency and specialized courses.

About canceling the internship

Before the changes in the medical education system that took place in 2016, medical graduates had to enter an internship, where they received practical skills in a general speci alty for a year. Instead of an internship, it was possible to study in residency for two years, choosing a narrow specialization. After a year of internship, the young specialist began independent work or continued his studies in residency.

According to Order No. 127n dated February 25, 2016, in the same year, they began to phase out the internship. The first stage concerned pharmacists and dentists, the second - doctorstherapeutic speci alties and surgeons. Now the graduate receives a diploma and an accreditation sheet, which together gives the right to conduct appointments in polyclinics, outpatient clinics as a therapist, pediatrician, general practitioner. For admission to residency, a year of working off is no longer required. The internship ceased to exist in 2023.

The abolition of internships, according to the Ministry of He alth, allows young people to start working earlier, include more practical training in the educational process, and attract young specialists to polyclinics and outpatient clinics. The effectiveness (or ineffectiveness) of the reform can be judged after several years after its implementation and publicity of the results.
Forms and stages of accreditation

The previous system of qualification confirmation by medical workers in 2023 was replaced by accreditation, which medical workers with medical and pharmaceutical education must pass. The new system in 2023 included specialists who had not previously undergone this procedure, graduates of medical universities and medical schools, specialists after completing residency and professional retraining.

Accreditation is divided into three forms, consisting of several stages:

    • primary are young professionals graduating from a medical and pharmaceutical university, college. Consists of a testing stage, assessment of practical skills, solving situational problems;
    • primary specialized medical workers are required to pass after professional retraining courses who have receivededucation abroad, knowledge assessment is carried out at three stages (as in primary);
    • employees periodically attend advanced training courses, portfolio is evaluated.

    On the test, consisting of 60 questions, 1 hour is allotted. If the correct answers are given to 70% (or more) of the questions, then the test is considered passed. Practical skills are assessed when working on simulators, mannequins, while solving problems related to a specific situation.

    All issues related to the accreditation of medical workers are spelled out in the Regulations on the accreditation of specialists, approved by order No. 334n of the Ministry of He alth of 06/02/2016 Changes in the education system (including residency) and retraining of medical specialists taking place in 2023 -2023, will be reflected in the Regulations.

    Residency - changes

    In order to obtain comprehensive knowledge necessary for the chosen profession, to be able to open a private medical practice or get a position in the state he althcare structure, it is necessary to complete residency training.

    Training options:

    • budgetary place can be obtained if the number of points exceeds the passing score set by the admission committee;
    • target - the course will be free, but the funds spent will need to be worked out in a state medical institution;
    • paid basis - for admission you need to score a set minimum points.

    Sometimes additional points play a decisive role for admission. ATIn 2023, the list of individual achievements for obtaining them has been expanded.

    Additional points are awarded for:

    • experience in the speci alty;
    • participation in the volunteer movement;
    • diploma of the Olympiad "I am a professional";
    • red diploma;
    • article in a specialized scientific journal;
    • if he received a presidential scholarship;
    • other individual achievements.

    Every year the number of budget places in residency is growing. For example, for 2022-2023 they were allocated by 13.8% more than in the previous year and amounted to 19,699. Most of them were distributed at the request of the regions.
